What is shared decision making?

"[...] a collaborative process in which patients and clinicians work together to make healthcare decisions informed by evidence, the care team's knowledge and experience, and the patient's values, goals, preferences, and circumstances."

What is patient-centered care?

Care in which "an individual’s specific health needs and desired health outcomes are the driving force behind all health care decisions and quality measurements."
